We’re excited to share that the application form for PhilHealth YAKAP Program (Yaman ng Kalusugan Program) — formerly known as PhilHealth Konsulta — is now available at iReply! 

This program gives PhilHealth members access to primary care clinics/hospitals, laboratory exams, medicines, and other health care screenings.

 🩺How to Apply:

• Fill out the YAKAP Application Form (the form is already good for two members).

• Provide your personal information and check all applicable boxes.

  • Select your preferred YAKAP-Konsulta Provider from the list: YAKAP Providers_Nationwide &  YAKAP Providers_Region VI and write it under “1st Choice KPP", choose one provider only (Example: Medisense Clinic and Laboratory Center Inc.)

• Leave the “2nd Choice KPP” and “Transfer” sections blank.

• Don’t forget to write your name and signature in the form.

• Scan and rename the accomplished form using this format:  YAKAP_Last Name, First Name

• Upload the scanned form to your cluster’s Google Drive folder.

• All iReply employees

• Family members of iReply employees who are PhilHealth members

• Use this format for dependents:

  •   YAKAP_Last Name of Employee, First Name of Employee_Dependent 1 (Dependent 2, etc.)
